How to get a meeting with Dr. Kullerd

  • Contact Golden Medical Care to Schedule a Meet & Greet

    • FREE, 15-minute Phone/Virtual appointment. 
    • Discuss why you are considering a new physician and your health goals.
    • Determine if Goldern Medical Care would be a good match for you. 

  • Schedule an Appointment with Dr. Kullerd

    • Contact Scheduling at 701.516.4673 ext 122
    • Complete patient agreement and consent of services form
    • Send in copies of your insurance cards
    • Complete forms and submit them prior to the initial appointment

  • Schedule your 90 minute comprehensive evaluation

    • Appointment location of client choice based on convenience (Home, facility, call, or video)
    • Appointment type adjusted to needs: Healthy brain aging, comprehensive geriatric cognitive assessment, preventive care, facility admission or follow-up, family conferencing etc

  • Elect Consultative Vs Ongoing Specialized Geriatric Primary Care with GMC

    • Consultative Assessment & recommendations are forwarded to your PCP, and you continue ongoing care with your Primary Care Physician 

    OR

    • Elect Ongoing  Premium Geriatric Care with GMC. Being accepted into the practice will depend on practice availability for new clients.
